Job Description
Summary
Performs physical therapy procedures delegated by a physical therapist to treat medically referred patients with physical disabilities. Assesses and reports progress or problems in attaining maximal function. Implements a treatment plan based on the physical therapist's evaluation and plan of care to address patient needs. Completes the appropriate documentation and billing in a timely manner. All care is provided in accordance with the philosophy and essential functions of the hospital, the rehabilitation department, the Indiana State Practice Act, the American Physical Therapy Association, and The Joint Commission.
Education
Must have an Associate's or Bachelor's degree from an accredited PTA program.
Licensure/Certification
Must hold or be eligible to receive a PTA license in the state where services will be provided within the first 90 days of employment. Must be certified in CPR or mus...
