Description

As a Transition Coordinator, you will coordinate transition services and instruction to enable students to reach their fullest potential and be able to live, learn and earn in their community as an adult. You will report to the Program Manager and will work onsite.

Responsibilities:


+ Work with NHS staff, Clinical staff, Parents/guardians, outside service providers and identified students to:

+ Design a realistic discharge plan that is guided by each youth’s wishes, hopes and dreams

+ Coordinate your client driven discharge planning process with local OCFS, OMH, and NYSED representatives that supports a client-driven plan which integrates housing, health and medical care, education and employment needs.
