Travel PT Home Health jobs in San Jose, CA covering Alameda County offer a 13-week contract with Monday to Friday 8-hour days and 40 hours per week. You will provide physical therapy to patients in their homes, working 25-30 points per week without overtime. Responsibilities include evaluating patient needs, developing individualized treatment plans, implementing therapeutic interventions, educating patients and caregivers, and documenting care using WellSky EMR.
