JOB DESCRIPTION

We are seeking a Texas licensed LPC or LCSW to join our behavioral health care management team as a Transition of Care Coach, supporting our Medicaid members with a behavioral health diagnosis and have been recently admitted to an inpatient unit. You will partner with the hospital’s discharge team from admission to discharge to make sure the transition from inpatient is successful. Preferred candidates will have previous discharge planning or case management within another MCO or an inpatient program.

Excellent computer skills and attention to detail are very important to multitask between systems, talk with members on the phone, and enter accurate contact notes.
