Catholic Health Home Care is looking for passionate, dedicated Speech Language Pathologists to provide comprehensive care to patients in Suffolk County including Bohemia, Brentwood, Fire Island, Holbrook, Islip, Lake Ronkonkoma, Ronkonkoma, Sayville, & surrounding towns.

Job Details
Our Home Health Speech Language Pathologists will:

+ Assess, plan, develop and provides the patient or responsible caregiver a home health plan of care for speech language pathology.

+ Identifies patient needs for other disciplines and services.

+ Coordinates and evaluates the plan of care with all other disciplines and physicians for an assigned caseload.

+ Instructs the patient or responsible caregiver on the projected care plan and expected outcomes as directed by the physician's treatment plan.

+ Detailed documentation of visits in EMR system.
