Job Description
Social Worker-(2300002I) Addresses social concerns of patients Performs psychosocial assessment and counseling therapy to patients, their families Provides follow-up services to appropriate patients and families. Participate in preparation of the treatment plan, especially for patients with psychosocial issue, and their treatment requires certain resources such as family support, transport, finance etc. Participate in preparation of the discharge plan to help patients to return to their home environment with optimal functioning condition Responds to referrals placed for inpatients, outpatient (RDU, special needs and kidney transplant patients) and ER, as per policy. - Follows up on patients for whom initial assessment is done when indicated.
