1. Job Summary
As a Social Worker at New Hope Community Services, you will play a vital role in driving our mission to bring change into the lives of the displaced and disadvantaged in our community. Your responsibilities will include conducting intake assessments, providing counselling and support, and facilitating access to external resources and services. The Social Worker works collaboratively with our dedicated cross-functional teams, community partners, and stakeholders, to be a Beacon of New Hope to the displaced and disadvantaged in our community. 2. Key Responsibilities
Engagement & Assessment Develop professional & engaged relationship with clients and other key service stakeholders (e.g. Famiy Service Centres (FSCs), Social Service Offices (SSOs), Ministry of Social and Family Development (MSF), etc) Conduct needs assessment and risk assessment for complex cases and refer to the relevant agencies for intensive case management.
