Working hours: Mon-Fri Office Hours

Salary: Up to $5000 (Depending on experience)

• Clinical Aspects
• Manage enquiries and referrals
• Conduct assessment and home/site visits, and provide counselling, behavioural interventions, groupwork and support forresidents and their families
• Conduct comprehensive assessments of residents' care and financial needs, and support in the submission of HOMES means testing and transport subsidy applications as required
• Look after clients' physical and emotional welfare, identify anomalies and provide the appropriate intervention or service linkage
• Manage emergency response and crisis management with regards to residents' behaviour management with the multi-disciplinary team
• Develop, implement and review Individual Training Plans forresidents with the multi-disciplinary team
