Job Description

Duties

:
  • Collaborate with SUCD Program teams and hospital/community partners (acute care, public health, withdrawal management, treatment programs) to coordinate care and follow‑up.
  • Provide evidence‑based assessments, consultations, and psychotherapy (individual, group, couples, family) for clients with complex concurrent substance use, mental health, and physical health concerns.
  • Conduct rapid risk assessments, develop crisis intervention plans, and respond effectively to acute situations (., suicide risk, intoxication).
  • Engage clients, families, and community supports in interventions, harm‑reduction strategies, and continuity‑of‑care planning.
  • Assess social determinants of health and support related needs (housing, transportation, medication coverage).
  • Utilize evidence‑informed therapeutic approaches (MI, CBT, DBT) within a trauma‑informed, harm‑reduction framework; facilitate concurrent disorder group therapy modu...
