Summary Social workers assigned to Health Care/Homeless Veterans (HCHV) provide psychosocial services and/or short-term case management to Veterans who are homeless/assist them in obtaining housing as expeditiously as possible. Once engaged through outreach, HCHV social workers connect Veterans to VA/community resources that provide prevention, supportive services and permanent housing.
