Job Summary:
In this position, the Emergency Department Social Worker provides comprehensive psycho-social assessment, management and support to at risk patients and families at Upstate Medical University Hospital. Knowledge of social determinants, health, and drivers of utilization are preferred. Duties will include consultation through assessment, counseling, intervention, planning and referrals to meet the psycho-social needs of patients and families. Candidate shall learn to initiate and implement advanced directives. The Social Worker will actively manage patients through multidisciplinary collaboration, coordination, assessment, identification of barriers to care and referrals for services to meet individual's health needs. The Social Worker rounds their assigned patients with the multidisciplinary team and participates in quality meetings on their assigned unit.
