Social Worker (BSW) Temporary Part-Time – Geriatric Emergency Management (GEM) Program

Sault Area Hospital is currently recruiting for a Social Worker (BSW) to join their team in the Geriatric Emergency Management (GEM) Program. This is a new role to the hospital. The Social Worker, embedded within the Geriatric Emergency Management (GEM) Program, plays a vital role in the holistic assessment and care planning of older adults in the Emergency Department (ED). The Social Worker is responsible for conducting psychosocial assessments, identifying risks and social determinants impacting care, and using validated tools to capture baseline functional status and cognitive abilities.

Working collaboratively with the GEM RN and interprofessional team, the Social Worker supports the early identification of patients at risk of functional decline and provides specialized psychosocial and system navigation expertise.
