Position Summary:



To provide assistance to people receiving services throughout all aspects of their lives across educational, residential and community environments. Provides support for activities of daily living (ADL’s) including personal care. Promote choice and independence while providing protective oversight. Demonstrate compliance with all Springbrook policies and State regulations to ensure safe and smooth operation of the Community Habiliation- Residential Program .



Primary Duties and Responsibilities:




+ Be a good role model for the people you support in all situations and settings

+ Follow all written plans, learning materials, and goals .

+ Read P articipant’s Individualized Service Plan, Community Habilitation Plan, Day Habilitation Plan, Participant Protection Guidelines and/or IPOP to gain a better understanding of the participant’s condition and needs.

+ Be an advocate for the people...
