Job Summary

We are currently looking to fill a Full‑Time opportunity for a Sexual Health and Blood Borne Infections Social Worker to support Population & Public Health in Abbotsford, B.C.

In this role, you will provide intensive, client‑centered HIV case management for individuals with complex care needs; conduct collaborative assessments, develop and monitor individualized care plans, and facilitate access to mental health services, referrals, and advocacy for clients and their families. You will use outreach approaches to engage at‑risk populations, ensure seamless connection to services along the continuum of care, and participate in case conferences with the broader SHBBI HIV team and other care providers.
