About the job Senior/Staff Nurse (Home Care)

The Home Care Nurse provides nursing care and services for elders in their homes across Singapore. Home care nurses also support SLEC daycare centres in providing centre-based nursing services for elders within the community.

Key Responsibilities:

• Perform patient assessment: Conduct comprehensive and holistic care assessment and review for clients care needs.
• Design care plans: Develop and regularly review Individualised Care plan (ICP) involving the caregivers / family members (if possible) for clients to meet care needs. Develop Discharge Plan for clients who do notno longer require the service.
• Perform nursing procedures: Perform wound management, stoma care, catheter changes, nasogastric tube changes, injections, blood taking, blood glucose monitoring, oral and tracheostomy suctioning.
