Role & Responsibilities Assist the Head and/or Counsellor to attend to referrals for placement, conduct intake and admission interviews
Use appropriate assessment tools to assess risks, needs, strengths of residents and work collaboratively with the Probation, Child & Adult Protective Services Officers to develop the Individualized Care Plan (ICP) with a view towards reintegration with family and with the larger community
Conduct in depth biopsychosocial assessment of vulnerable families, design and deliver client centric interventions to vulnerable families
Ensure that the ICP is carried out with appropriate and sound intervention plans
Conduct regular meetings with relevant partners and stakeholders to evaluate resident's progress
Plan, design and conduct rehabilitative and developmental groupwork programmes
Maintain up-to-date case records and progress records of residents
