Job Description
Responsibilities
- Conduct phone screenings to collect biodata and gather social information for medical social workers. Contact patients for appointment reminders and rescheduling.
- Accompany medical social workers on field visits or conduct them independently.
- Timely completion of case recordings.
- Liaise with external agencies.
- Transcribe applications onto online referral systems.
- Monitor and follow up on referral statuses.
- Handle external correspondences.
- Attend to Medical Social Services enquiry emails.
- Maintain work statistics and perform clerical duties.
