About the role

CALHN Integrated Care provides services to individuals who do not require the resources of a general hospital but are beyond the scope of the traditional primary health care services. The focus is on preventing unnecessary hospital admissions and facilitating early discharge pathways from the acute sector.

In this role, the pharmacist will have opportunities to work across the Integrated Care service including SA Urgent Care Hubs, Geriatrics in the Home and Hospital in the Home.
