Palliative Care Provider North-East/Central Location Be Part of An Integrated Healthcare Team
Reporting to the Day Care Manager, you will be responsible for providing excellent individualised rehabilitative care to patients in Day Care, Homecare and Inpatient to optimise their quality of life.
Responsibilities : Manage clinical caseload and support palliative rehabilitation practice (e.g. breathlessness management, prescription of equipment, collaborative goal setting) across the various care settings. Deliver evidence-based individualised care through holistic comprehensive assessment, treatment and evaluation of care. To work collaboratively with multi-disciplinary colleagues to meet patient needs. Participate in service review to drive service improvement.
