Overview

The purpose of this role is to take a lead within the wider multidisciplinary team to effectively assess and monitor the physical health of patients who receive treatment that requires monitoring and who live in the community. The team will facilitate and/or deliver evidence-based interventions as needed to deliver safe care and improve health.

Responsibilities

• Work closely with the Clinical Team Manager (CTM) in overseeing efficiency and effectiveness of care provided to a caseload of patients.
• Present as a positive and professional role model to all staff within the unit, striving to maintain a philosophy of service user involvement in continuously improving the service.
