About the job Senior / Medical Social Worker
- Palliative Care Provider
- Part of Interdisciplinary Specialist Palliative Care Team
- Collaborative Working Environment
- Provide and facilitate socio-economic evaluations to help patients and their families address practical concerns, such as financial matters, which are often an issue at the end of life
- Ongoing individual and family counselling through face-to-face sessions or home visitations in order to help navigate the personal and ethical dilemma, cultural conflict and issue relating to religion, spirituality and the meaning of life
- Work with the broader community which includes educational, religious, and social service organizations so that patients and their families can achieve optimal end of life goals
