**UW Medical Center-Montlake - Social Work & Care Coordination** has an outstanding opportunity for a **Nurse Care Coordinator** .
**WORK SCHEDULE**
+ Full time / 100% FTE
+ Day shifts, Monday - Friday
**POSITION HIGLIGHTS**
+ Nurse Care Coordinators work closely with all members of the interdisciplinary health care team, ensuring the team is aware of the patientβs plan of care
+ This position will specifically provide care coordination for transplant, orthopedic, and medical-surgical patients
**PRIMARY JOB RESPONSIBILITIES**
+ Identification and monitoring of high medical needs patients to ensure timely completion of consults, procedures and tests, ongoing team communication, and facilitates coordination between providers across the continuum of care.
+ Lead medical team and nursing huddles and work creatively on removing barriers to discharge due to medical complications and/or system or tea...
