Serenity Hospice is looking to add a dedicated and compassionate RN Case Manager to delivering exceptional care, comfort and support to patients. They will develop plans for individual needs, perform regular check-ups and communicate with family members as needed. This position will oversee the Warren County area.

Responsibilities:


+ Complete an initial assessment of patient and family to determine hospice care needs

+ Regularly re-evaluates the patients nursing needs
