Advocate Trinity - Inpatient Care Manager

o Level II NICU
o No trauma designation
• Unit Information:
o Beds: 205 is hospital, unit sizes vary
• Census? Average patients/day?
o Minimum Years of Experience required? 3 years of acute care case management experience
o Will you accept a first-time traveler? Yes
• Patient Types/Common Diagnoses:
o This is an underserved population, mostly adults and geriatrics. Will see complex cases like unhoused and low-income patients
o Could be assigned to PCU, MedSurg, LDRP; will likely not be assigned ICU but possible
o Will be assigned to a specific unit and could float to cover other units
• Case Load:
o Typically, around 14-20 patients
• Required License, National Certification, Certs:
o No specific certifications required. Will accept ASN or higher
