As an Outreach Team Member for the Enhanced Care Management program, you will identify, contact, and engage eligible individuals to participate in our free healthcare services. Your role is crucial in helping vulnerable populations access comprehensive care coordination that can significantly improve their health outcomes.

• Identify and reach out to individuals who qualify for service programs
• Clearly explain program benefits and services in an accessible, compelling manner
