About the role

Hillmorton Hospital, Christchurch. Wāhi Tautoko is a 10‑bedded service located in Building 8. The service provides extended assessment, treatment and rehabilitation for consumers who need additional care following admission to adult acute services. It supports people in managing symptoms, transitioning into the community, building social supports, and creating safe discharge plans.

Responsibilities

• Work closely with an experienced multidisciplinary team to deliver person‑centred mental health care.
• Provide extended assessment, treatment and rehabilitation for consumers after acute inpatient admission.
• Support consumers in illness management, life skills and development of meaningful routines.
• Facilitate safe discharge into a community setting and support transition into home or supported accommodation.
• Teach skills and collaborate with consumers to achieve the life they want.
