Summary This Registered Nurse- Case Manager is in the Connected Care service at the Togus VA Medical Center, located at the Augusta, Maine location. This position is a full-time position at 40 hours per week. Responsibilities The Registered Nurse- Case Manager focus is on individual growth and development in practice with increased levels of self-direction. Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings. The Registered Nurse- Case Manager assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs.
