+ Provides in-home visits to home health clients within the agency’s service area.

+ Conducts OASIS assessment and documents information in the automated clinical record.

+ Serves as case manager for home care clients using assessment data to develop a plan of care based on evaluation of identified problems.

+ Educates patients and families for home care and discharge planning needs. Provides appropriate transfer or discharge instructions/information to patient and family.
