About the Role

As a Registered Health Professional (Social Worker or Occupational Therapist) you will play a key role in supporting older people and their whānau through their mental health and recovery journey.

• Undertake comprehensive assessments and develop tailored care or rehabilitation plans.
• Deliver interventions that promote independence, wellbeing, and quality of life.
• Support people living with dementia in both community and residential care settings.
• Work collaboratively with whānau, carers, and other providers to improve outcomes.
• Contribute to meaningful, person‑centred goals through therapeutic engagement and rehabilitation.
• Provide care coordination and support functional rehabilitation and environmental adaptations.
• Work as part of a collaborative, multidisciplinary team to deliver safe, effective, and integrated care.
