You will collaborate with the Food Service Manager and the care team to assess and plan nutritional care for residents. Key Responsibilities:
• Assess and plan individualized nutritional care for residents
• Consult on hydration and nutrition program needs
• Approve seasonal menus for nutritional adequacy
• Educate residents and families on nutrition recommendations
• Participate in quality indicator committees and interdisciplinary teams
