Summary The Nurse Housing and Urban Development - Veterans Affairs Supportive Housing (HUDVASH) Registered Nurse (RN) is responsible for providing competent, evidence-based care to assigned patients. Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings. Assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs. Responsibilities The HUDVASH RN develops a plan of care using information from the patient, family, and provider. Bridges the gap between discharge and primary care follow-up. Identifies high-risk patient population and transitions to appropriate discipline (i.e., Primary Care, Home Based Primary Care, Hospice/Palliative Care etc.).
