This team has established strong integrated workflows under the patient center medical home model. Work with a diverse patient population with complex medical and mental health needs. Experience with medicated assisted therapy (MAT) and chronic pain patients preferred. Enjoy a healthy work-life balance in the west suburbs of Portland.
* Monday to Friday schedule
* Ideal candidate will have experience with primary care behavioral health care, including consultations, warm hand-offs and brief interventions for individual treatment
