Summary The position serves as a Program Support Assistant to an assigned Home-Based Primary Care (HBPC) PACT team, under the supervision of the HBPC Program Director. The primary purpose of this position is to provide assistant level support to the Program Director, Nurse Manager, Assistant Nurse Manager, Medical Director and the HBPC interdisciplinary staff. Responsibilities The incumbent provides administrative services to Veterans such as determining the eligibility of patients for admission to the HBPC program, assisting with administrative completion of forms or documents, etc., as well as assisting other HBPC PACTs as the need arises. The incumbent plays an integral role in identifying and developing ways to resolve problems with the administrative and clinical staff on customer service, patient flow, revenue and data validation issues.
