About the Role
Supported Transfer and Accelerated Rehab Team (START) is an early supported discharge programme that accepts referrals for patients who are medically cleared and can be discharged home earlier than previously possible with an individualised care plan and intensive holistic support. START provides up to four visits a day, 7 days/week across Rotorua & Taupo using Health Care Assistants with oversight from Registered Nurses, Physiotherapist, Occupational Therapist and input as required from Medicine / Geriatricians and other specialties e.g. SLT.
