As one of the nation’s oldest community health centers, we are deeply committed to delivering exceptional, patient‑centered care to the communities we serve. Our Physicians, APRNs, and PAs work collaboratively to help patients achieve their health and wellness goals.
We offer opportunities to teach Yale residents and medical students—with corresponding clinical faculty appointments—as well as the option to incorporate Telehealth into your practice.
