POSITION SUMMARY:

Under the Supervision of the Manager of Rehabilitation Services, evaluates and provides direct patient care per physician orders to rehabilitate patients in an outpatient setting to achieve stated goals. Services are provided according to hospital and departmental policies and procedures in compliance with regulatory standards.



Physical Therapist Key Responsibilities:


+ Completes a full initial evaluation on each patient referred appropriate to the patient’s diagnosis according to established procedure and physician order.

+ Provides timely documentation at the time of evaluation

+ Provides clear daily and progress reporting

+ Instructs and assesses patients in a variety of functional activities.
