Job Description
**Job Summary and Responsibilities**
As our Physical Therapist, you will help Hospice/Home Healthcare patients perform all phases of physical therapy evaluation, treatment planning, treatment, and documentation so they can achieve their rehabilitative goals and improve their quality of life in a home setting.
Every day you will evaluate patients, participate in treatment program planning, and administer physical therapy modalities, procedures, and tests.
To be successful in this role, you must have at least two years of professional experience in rehabilitation, maintain compliance with professional standards of conduct, and effectively communicate patient progress to the medical team.
+ Perform patient initial and re-evaluations, establishing and implementing effective treatment plans.
+ Administer treatment plans using accepted therapy procedures, overseeing Certified Home Health Aides as therapist extenders.
