Responsibilities:
* Provide skilled rehabilitation services to patients as outlined in the Plan of Treatment
* Evaluate, plan, and administer physical therapy modalities to develop and restore function
* Deliver rehabilitative physical therapy in patients' homes as prescribed by a physician
Qualifications:
* Current PT licensure in Georgia with no sanctions
* Strong communication and documentation skills
* Experience in Home Health is preferred but Not required
