Job Description
All Care and Comcare Home Health is currently seeking a qualified Physical Therapist to join our dedicated healthcare team. Position: Field Physical Therapist
Key Responsibilities:
- Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders.
- Perform start of care (SOC) evaluations, resumption of care (ROC), recertifications (Recert), and discharges (DC).
- Assess patient skilled needs, plan interventions, evaluate outcomes, and modify care to meet physical therapy goals.
