Job Description
Clinical Pharmacist
This onsite Clinical Pharmacist role focuses on providing specialized patient care in a non-retail pharmacy setting. You’ll spend much of your day counseling patients by phone, reviewing therapies, and working closely with care teams to ensure the best outcomes.
What You’ll Do
+ Conduct detailed reviews of patient medications (interactions, adherence, side effects, dosing, etc.)
+ Counsel patients on therapies and answer questions (primarily phone-based)
+ Document clinical interventions and updates in patient charts
+ Collaborate with physicians, nurses, and care teams to coordinate patient care
+ Provide education on medications, including injection support when needed
+ Ensure compliance with REMS programs, pharmacy regulations, and safety standards
+ Verify prescriptions and support accurate dispensing processes
+ Assist with intake, patient onboarding, and ongoing care coordination
