We are seeking a patient-focused Pharmacist with strong counseling experience to join a fast-paced, clinical environment. This role is highly interactive, requiring extensive time on the phone providing patient education and support. Key Responsibilities
Patient Care & Clinical Duties

+ Counsel patients by phone on medication therapy, usage, side effects, and adherence

+ Conduct comprehensive clinical reviews, including drug utilization, interaction screening, and therapy optimization

+ Provide education on new drug therapies and support patient understanding of treatment plans

+ Document pharmacotherapy reviews and patient interactions in clinical records
