About the Opportunity

Under the direct supervision of the Clinical Pharmacy Manager responsible for the Cardiology & Hearth Health team, the Pharmacist will provide direct pharmaceutical care to inpatients and outpatients as a member of the interdisciplinary care team. Responsibilities include patient counseling, medication monitoring, student mentoring, drug utilization reviews, liaison with community pharmacists, education and drug information. The Pharmacist will also participate in clinical pharmacy research and quality improvement activities in alignment with the Pharmacy Department.
