Job Description
Overview
The AMS pharmacist provides direct patient care including management, monitoring, and instruction related to anticoagulation management. The pharmacist will provide high quality patient care through a collaborative practice agreement for anticoagulation management of patients referred by any Carle Provider to AMS. They will be responsible for implementation, monitoring, and overall management of anticoagulation therapy regimens. They will provide extensive patient education on warfarin therapy. They will work to ensure appropriate and optimized anticoagulant therapy and bridging, and provide education to patients as well as other healthcare professionals. They will provide direct supervision and instruction to AMS pharmacy technicians.

Certifications: Licensed Pharmacist - Illinois Department of Financial and Professional Regulation (IDFPR), Education: Bachelor's Degree: Pharmacy, Work Experience:
