**Patient Placement Coordinator RN in Anaheim, CA. This position is Full-time and will work 10-hour, Variable shifts.**
Patient Placement Center Nurse supports care for Providence St Joseph Health (PSJH) patients by evaluating and coordinating the patient intake process. He/she will be involved in taking and triaging calls, and facilitating inbound and outbound transfers for PSJH facilities. In this role, he/she will act as a liaison between physicians, hospitals and other care providers who seek to refer, schedule admission or transfer patients into the hospital. He/she will ensure timely medical communication for patient referrals and transfers to/from the facility. He/she shall employ critical thinking to integrate objective data with knowledge gained from preliminary/initial information with the ongoing assessment process, including the objective and subjective experiences and patient health care history for optimal placement.
