Job Description

:The Obstetrical Care Navigator collaborates with the multidisciplinary team in the ambulatory care setting, working directly with patients to support the patient’s care plan in order to help assist them with social determinates of health, behavioral health and substance use issues that might be affecting their pregnancy and improve their overall health outcomes.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Coordinates the care and services for patients identified as needing assistance or meeting Care Management criteria based on screening tools and provider and nursing referrals. Collects in-depth information about a patient’s medical, functional, and social condition, to identify individual needs in order to develop a plan to meet those needs. Works with patient and family/caregivers to determine specific goals and actions based on assessment.
