Job Description
RESPONSIBILITIES
I. JOB SUMMARY/RESPONSIBILITIES:
β’ Supports community health by establishing and maintaining trusting relationships with patients, families, and providers to promote health, recovery, resiliency, and wellness.
β’ Performs activities off-site, in the community, and makes patient home visits as required.
β’ Performs duties and responsibilities in accordance with policies and procedures and consistent with the goals and objective of the department and the Queenβs University Medical Group (QUMG).
II. TYPICAL PHYSICAL DEMANDS:
β’ Essential: stooping/bending, finger dexterity, seeing, hearing, speaking, lifting usual weight of 10 pounds up to 25 pounds.
β’ Frequent: standing, walking, reaching at and below shoulder level.
β’ Occasional: sitting, kneeling, climbing stairs, walking on uneven ground, squatting, pushing/pulling usual weight of 1 pounds up to 2.5 pounds of force, carrying usual weight of 5 pounds, reaching above shoulder level.
