Job Description
RESPONSIBILITIES
**Position Summary**
The **Patient Care Coordinator (RN)** is a key member of the multidisciplinary transplant team responsible for coordinating complex patient care across the transplant continuum, including evaluation, waitlisting, transplantation, and long‑term follow‑up.
This is a **non‑bedside, coordination‑focused nursing role** well suited for nurses with strong clinical judgment, chart review skills, computer proficiency, and patient relations experience.
**Work Schedule & On‑Call Expectations**
+ This is a **weekday, daytime role** (Monday–Friday, generally standard business hours).
+ The position includes participation in a **shared on‑call rotation** , with coverage distributed among team members and supported by established processes.
