**Position Overview**

As an LPN in the Day Habilitation/Group Home program, you will:

+ Provide essential medical care to individuals with disabilities
+ Maintain accurate health records and documentation
+ Advocate for the well-being of those you serve
+ Collaborate closely with the House Manager and care team

**Schedule (16 hours/week)**

+ Week 1: Sunday, Wednesday | 11:00 PM – 7:00 AM
+ Week 2: Wednesday, Saturday | 11:00 PM – 7:00 AM
