There is an immediate opportunity for the right Nurse Practitioner to provide part-time care in an on-site health and wellness clinic located near Burlington, KY for 25 hours a week (Monday, Wednesday & Friday). Through a coordinated care model in the workplace and a comprehensive suite of services; direct primary care, wellness, and occupational medicine— the clinician will create a greater continuum of care and make an impact on the health of the workforce.

What a Mid-Level Practitioner can expect from this position:

Face to face clinical encounters with employees, spouses, and dependents over 3 years old. Direct Primary Care, occupational injury assessment & treatment, as well as disease management and wellness visits.

Medical oversight provided by a local Medical Director and Chief Medical Officer

Work closely with the employer on work injury cases

On-Call 4-5 days per month
