**Description:**

The Nurse Navigator is a key member of the multidisciplinary team functioning as an advocate, educator and facilitator of care. Responsible for ensuring all referred patients receive quality and comprehensive services throughout the continuum. Coordinates the overall subspecialty programmatic goals and outcomes.

**What the Nurse Navigator will do:**

+ Completes a comprehensive health assessment of the patient’s physical, psychological, social, environmental, financial and

+ Assists in organizing and coordinating timely patient appointments.
